By submitting this online request I accept full title and risk to the equipment that TDS issued under the service contract I hereby want to terminate. I warrant to indemnify and hold harmless TDS from any claim in any way related to the equipment. My warranty includes indemnification from claims for compensation for material damage and/or compensation for personal injury which occurs as a result of the antenna hitting and/or contacting any object and/or person; When service is terminated the Customer is entitled to be refunded his/her deposit with TDS but TDS will set off all outstanding balances before funds are transferred to customer's bank account specified above. In case after this settlement the customer still has an outstanding balance with TDS, a final invoice will be produced. Customers with an active bundle that includes the TDS Service will be contacted by the UTS/Flow Team prior termination of the TDS Service.
